Superbill

An itemized form listing the services, codes, and diagnoses from a visit, used for charge entry or for patient reimbursement claims.

Also called: encounter form, charge slip, fee ticket

Definition

A superbill is an itemized record of a visit that lists provider details, procedure and diagnosis codes, units, and charges. Practices use it as a charge capture tool. Out-of-network patients often submit it to their health plan to seek reimbursement themselves.

Why it matters

Paper or templated superbills go out of date when code sets change each year. An outdated form steers providers toward deleted or vague codes, which causes rejections. A superbill given to a patient also needs an accurate NPI and tax ID, or the plan may reject it.

Example

An out-of-network therapy practice gives each patient a monthly superbill. Patients send it to their plan with a claim form to request reimbursement.
