TruCare vs Athelas: Revenue Operations vs an AI EHR With Billing

Athelas, now part of Commure, sells an AI EHR, an ambient scribe, voice agents, and managed billing to independent practices and specialty groups. TruCare runs intake, billing, and credentialing on one platform that works next to the EHR you already use. The right pick depends on whether you want to change your EHR.

How they differ

  • Platform: Athelas bundles its own AI EHR, called Air, with RCM managed services. TruCare connects to 20+ EHRs, so your clinicians keep the charting system they know.
  • Credentialing: TruCare includes credentialing and payer enrollment through TruCred. We found no credentialing service on the Athelas site.
  • Front end: Both cover intake and eligibility. Athelas adds voice agents for scheduling, and TruIntake handles scheduling, intake, and eligibility checks.
  • Pricing: Athelas does not publish pricing. TruCare publishes rates from 2.49% of collections.

Where they overlap

Both take day-to-day billing work off your staff. Each covers eligibility checks, claim submission, denials, A/R follow-up, and payment posting, and both pair automation with people who review the exceptions. Both also work with specialty practices, so the deciding factor is usually your EHR.

Choose TruCare if

You want one team for intake, billing, and credentialing, you plan to keep your current EHR, or you are adding providers who need payer enrollment handled before their first visit.

Choose Athelas if

You want to replace your EHR with an AI-first one and hand billing to the same vendor. Athelas says more than 90% of its claims need no human touch, and it publishes service-level commitments for its RCM team, such as submitting 95% of claims within 24 hours of closing.

Questions to ask on your demo

  • EHR: Can we keep our current EHR, and if not, what does migration cost in time and training?
  • Credentialing: Who enrolls new providers with payers before their first visit?
  • Reporting: What does a typical monthly report show about denials and A/R?

The bottom line

If your EHR works and you need credentialing too, TruCare is the simpler change. If you already plan to replace your EHR, Athelas lets you buy charting and billing together. Ask both for a sample monthly report before you decide.
