R1 is an enterprise revenue cycle partner for hospitals and health systems, taken private by TowerBrook and CD&R in November 2024. TruCare is built for independent practices, medical groups, MSOs, and digital health companies. They rarely compete for the same buyer, but the overlap grows as groups scale.
How they differ
- Buyer: R1 says it works with 95 of the top 100 US health systems. TruCare focuses on outpatient practices and groups.
- Scope: R1 runs end-to-end and modular outsourcing on its Phare platform, including hospital coding and underpayment recovery. TruCare covers intake, billing, credentialing, and analytics for ambulatory care.
- Credentialing: TruCred is a core TruCare module. We found no credentialing product marketed by R1.
- Pricing: R1 uses custom, outcome-linked partnership terms. TruCare publishes rates from 2.49% of collections.
Where they overlap
Both take ownership of revenue cycle work rather than only selling software. Both use automation and AI, and both report outcomes like denial rates and days in A/R back to the client.
Choose TruCare if
You run a practice, a medical group, or an MSO and want a partner sized for you, with credentialing and published pricing included.
Choose R1 RCM if
You run a hospital or health system and want to outsource most of the revenue cycle to one large partner. R1 earned 2026 Best in KLAS ratings in 3 services categories, including extended business office for smaller hospitals.
Questions to ask on your demo
- Fit: What is the smallest organization you serve today, and how many clients look like us?
- Credentialing: Is provider enrollment included, and who runs it?
- Pricing: How is the fee tied to results, and what is the minimum contract?
The bottom line
For a hospital or health system, R1 is a natural shortlist pick. For a practice, group, or MSO, TruCare offers a closer fit, credentialing on the same platform, and pricing you can see up front. Enterprise contracts are often larger than an ambulatory group needs.
