Speech Therapy
billing and RCM services

Speech therapy spending does not stand alone. Medicare adds it to the same patient’s physical therapy charges under one shared cap, and crossing it changes what every claim needs. Our speech therapy billing services track that number correctly, for every patient, every time.

We were not ready for the 92507 proposal
We found out about the proposed change from a denial pattern, not from anyone warning us ahead of time. That is not how we want to find out about anything again.

Our combined threshold stopped surprising us
We were tracking our SLP spending alone and had no idea it was being added to our patients’ physical therapy charges toward the same number.

Our AAC claims finally sequence correctly
We were billing the device programming code without the evaluation code behind it, and it denied every single time.

Our swallowing studies stopped getting lost between departments
Coordinating with radiology on videofluoroscopic studies used to mean claims sitting in limbo. That handoff finally works.

The code you bill constantly is the one about to change the most.

Most specialties see their core codes update gradually, a value adjusted here, a modifier clarified there. Speech-language pathology is facing something far more disruptive. The single code most SLPs bill more than any other, the workhorse of individual treatment, is the subject of a formal federal proposal to delete it entirely and replace it with a set of diagnosis-specific codes instead. A change like that does not update how a practice bills. It replaces the foundation underneath it.
Layered on top of that sits a threshold most SLPs do not realize they share. Speech therapy spending counts toward the same annual Medicare limit as physical therapy, for the same patient, and a practice that tracks its own speech therapy spending in isolation is working from an incomplete number.
Then there is a service category that behaves nothing like standard therapy billing at all. Augmentative and alternative communication devices are evaluated and programmed under specific SLP codes, but the device itself can separately function like a piece of durable medical equipment, with its own trial period and its own documentation trail.
A billing company unfamiliar with these specific, current realities will bill today’s claims correctly and be caught flat-footed the moment the ground underneath them actually shifts.
Six places speech therapy revenue goes missing
No plan exists for the proposed code change
A practice that waits until a new code structure takes effect to understand it will spend its first claims of that period correcting avoidable denials instead of collecting revenue.
The combined threshold was tracked as speech-only
Billing as though speech therapy spending exists in isolation from a patient’s physical therapy charges produces a number that does not match what Medicare is actually tracking.
AAC device services were billed out of sequence
The ongoing programming and therapy code depends on the evaluation code being on file first. Billed without it, the claim fails regardless of the work performed.
A swallowing study claim was missing its radiology half
A videofluoroscopic study requires documentation from both the SLP and the imaging side, and a claim missing either one does not stand on its own.
Multiple evaluation codes were billed without separate justification
Billing more than one of the distinct evaluation codes the same day without documentation supporting each as its own service invites a denial.
The GN modifier was missing
Without it, a claim does not identify itself as a speech-language pathology service under a plan of care, and processing does not proceed correctly.

Our service offerings

Every part of a speech-language pathology practice

Individual and group treatment
Core therapy sessions billed under their current code, with our process already built for the transition those codes are expected to undergo.
Speech, language, voice, and fluency evaluation
Four distinct evaluation codes, each requiring its own documentation when more than one is billed on the same date.
Swallowing and feeding therapy
Treatment of dysphagia, billed separately from standard speech treatment and tied to its own supporting diagnosis.
Swallowing evaluation, including instrumental studies
Clinical bedside evaluation, videofluoroscopic studies coordinated with radiology, and endoscopic evaluation, each billed under its own distinct code.
AAC and speech-generating device services
Evaluation for device prescription, billed by time, followed by ongoing programming and therapeutic use of the device once it is in the patient’s hands.
Cognitive-communication therapy
Treatment for cognitive-linguistic deficits following stroke or brain injury, billed under the codes that reflect this specific focus.
Remote therapeutic monitoring
Newer codes supporting monitoring of a patient’s home communication or swallowing program, billed alongside standard treatment where applicable.
Skilled nursing facility-based speech therapy
Treatment delivered in a SNF setting, billed under the specific Part A and Part B rules that apply there, distinct from outpatient billing.

What actually makes this work

Billers who track the shared threshold, not just your own number
Certified billers on your account combine speech therapy spending with physical therapy spending for the same patient, the way Medicare actually calculates it.
Already preparing for a code change that has not taken effect yet
We are tracking the proposed 92507 deletion now, so your practice is not learning a new code structure from its own denials when the change arrives.
No disruption to how your practice already runs
We work from the system your practice already uses. Nobody learns a second one.

What actually causes speech therapy claims to fail

A speech therapy denial is rarely about whether treatment was appropriate. It is almost always about sequencing, a shared threshold calculated incorrectly, or documentation split across two departments that never quite connected.
Here is what we see most, and how each is caught before it becomes routine.
Curious what your denials are actually costing you?

The recurring six:
Therapist using a tongue depressor on a smiling girl
Multiple evaluation codes same day, no separate documentation
Billing more than one distinct evaluation code without justifying each independently is a routine, avoidable denial.
AAC therapy billed without its evaluation code on file
The ongoing device service depends on the evaluation code having already been submitted, and billing it out of order fails.
Missing GN modifier
A claim without this modifier does not correctly identify itself as an SLP service delivered under a plan of care.
Swallowing study missing radiology documentation
A videofluoroscopic study claim incomplete without its imaging-side documentation does not stand on its own.
Combined threshold miscalculated
Tracking speech therapy spending alone, separate from the patient’s physical therapy charges, produces an incorrect number relative to what Medicare is actually tracking.
CPT and diagnosis mismatch
A treatment code billed without a diagnosis that actually supports it, such as a dysphagia code paired with a general speech diagnosis, does not meet documentation expectations.

The codes your speech therapy revenue runs on, and where they are headed

Our speech therapy billing and coding services run on today’s code set while actively tracking where that code set is headed. Our coders check sequencing, shared threshold accrual, and documentation completeness before any claim goes out.

We connect to the EHR you already use

A speech-language pathology practice documents through its own charting system, and we do not ask that to change. Whether your practice runs on WebPT, Clinicient, or hellonote, TruCare pulls from whatever platform your practice already uses.
What changes is what gets caught before it becomes a denial. An AAC therapy claim about to go out before its evaluation code is on file. A combined threshold being tracked as speech-only. A swallowing study missing its radiology documentation. Each one surfaces while it can still be corrected.

What actually moved for this specialty this year

CMS formally proposed deleting the most-used treatment code in the specialty
In July 2026, CMS proposed deleting CPT 92507, the generic individual treatment code speech-language pathologists have relied on for decades, to be replaced with ten diagnosis-specific codes effective in 2027. This is a proposal, not yet a final rule.
The combined therapy threshold rose
For 2026, the KX modifier threshold for combined physical therapy and speech-language pathology spending rose to $2,480, continuing to accrue as a single shared number rather than being tracked separately by discipline.
New remote therapeutic monitoring codes became available for SLPs
Codes 98984, 98985, and 98979 were introduced, billable with the GN modifier, supporting monitoring of a patient’s communication or swallowing program outside of a direct treatment session.
Telehealth authority for core SLP services was extended
Coverage for telehealth delivery of core codes, including 92507, 92508, 92521 through 92524, and 92526, was extended through December 31, 2027.

Someone has to track a proposal before it becomes a rule

A code deletion proposed in the middle of one year and taking effect the next is exactly the kind of change that catches an unprepared practice during its first weeks of a new billing cycle.

We track the proposal now, not after it becomes final, and we will build your claims around the new structure as soon as it is confirmed.
In-house vs. outsourced

What one biller cannot track through a code transition like this

A general biller can learn today’s speech therapy codes well. Tracking a shared threshold calculated across two disciplines, sequencing AAC billing correctly, and preparing for a proposed deletion of the specialty’s core code before it even takes effect is a different, more forward-looking kind of attention. Outsourcing speech therapy billing services puts people on this who are already watching where the rules are headed, not just where they stand today.

Speech therapy billing saver guide

Example: a practice collecting $710,000 a year with one full-time biller, at TruCare’s starting rate of 2.49%. Biller cost uses the BLS May 2025 median wage for medical records specialists ($51,140), with benefits at 30% of total compensation. Software and training costs vary by practice and are not counted. We do not replace your front desk, we take the claims work off it. Larger practices are quoted on volume rather than at the starting rate, so a bigger practice should not scale this example up.

What a practice gains once it is ready for the change before it arrives

“We found out about the proposal to delete our most common code from a billing newsletter, almost by accident. It made us realize we had no actual plan for what happens when the code we use on nearly every claim simply stops existing. Having someone already tracking that before it becomes official changed how we think about the next year entirely.”
