What HCPCS codes do in home health billing
On a physician claim, HCPCS and CPT codes usually determine payment line by line. Home health works differently. Under the Patient-Driven Groupings Model (PDGM), Medicare pays a 30-day period amount based on the HIPPS code, not on the individual visit codes. So why do home health HCPCS codes matter so much?
Three reasons. First, the visit lines prove which disciplines saw the patient and when. Second, Medicare counts those visits to decide whether a period falls below its low-utilization payment adjustment (LUPA) threshold, which ranges from two to six visits. Third, visit units and costs feed outlier calculations. If visit coding is wrong, a period can be paid as a LUPA when it should have paid in full, or the claim can return to provider.
This guide covers the Level II HCPCS G-codes used on Medicare home health claims, how they pair with revenue codes, and the reporting rules that trip agencies up.
How a home health visit line is built
Each visit on a final claim (type of bill 0329) is a separate line with:
A revenue code for the discipline (for example, 055X for skilled nursing).
A HCPCS G-code describing the type of visit and who performed it.
A line-item date of service.
Units reported in 15-minute increments, following Medicare Claims Processing Manual Chapter 10 rules.
Charges.
The first line of the claim is different. It reports revenue code 0023 with the five-character HIPPS code that identifies the PDGM case-mix group. HIPPS codes are a separate code set from HCPCS, even though they share a claim.
Skilled nursing HCPCS codes (revenue code 055X)
HCPCS | What it describes |
|---|---|
G0299 | Direct skilled nursing services provided by a registered nurse, each 15 minutes |
G0300 | Direct skilled nursing services provided by a licensed practical nurse, each 15 minutes |
G0493 | RN skilled observation and assessment of the patient's condition, when a change in condition needs skilled evaluation |
G0494 | LPN skilled observation and assessment of the patient's condition |
G0495 | RN training or education of the patient, family or caregiver |
G0496 | LPN training or education of the patient, family or caregiver |
G0162 | RN management and evaluation of the plan of care, when the patient's overall care needs a skilled professional to manage |
Choosing between these codes is not cosmetic. G0493 should be supported by documentation of a real possibility of change in condition that needs skilled assessment. G0162 requires that the combination of unskilled services needs RN oversight to be safe and effective. G0495 should show what was taught and how the patient or caregiver responded. Auditors compare the G-code with the visit note, so match the code to what actually happened.
Therapy HCPCS codes
Discipline | Revenue code | HCPCS | What it describes |
|---|---|---|---|
Physical therapy | 042X | G0151 | Services of a qualified physical therapist, each 15 minutes |
Physical therapy | 042X | G0157 | Services of a qualified physical therapist assistant, each 15 minutes |
Physical therapy | 042X | G0159 | Physical therapist establishing or delivering a safe and effective maintenance program |
Occupational therapy | 043X | G0152 | Services of a qualified occupational therapist, each 15 minutes |
Occupational therapy | 043X | G0158 | Services of a qualified occupational therapy assistant, each 15 minutes |
Occupational therapy | 043X | G0160 | Occupational therapist establishing or delivering a maintenance program |
Speech-language pathology | 044X | G0153 | Services of a qualified speech-language pathologist, each 15 minutes |
Speech-language pathology | 044X | G0161 | Speech-language pathologist establishing or delivering a maintenance program |
Maintenance therapy codes (G0159, G0160, G0161) reflect the Jimmo v. Sebelius settlement principle: Medicare covers skilled therapy to maintain function or slow decline when the skills of a therapist are needed, not only when the patient is improving. Documentation must show why a skilled therapist, rather than an aide or caregiver, was required.
Social work and aide HCPCS codes
Discipline | Revenue code | HCPCS | What it describes |
|---|---|---|---|
Medical social services | 056X | G0155 | Services of a clinical social worker, each 15 minutes |
Home health aide | 057X | G0156 | Services of a home health or hospice aide, each 15 minutes |
Aide visits count toward the LUPA threshold, but aide services are only covered when the patient also needs a qualifying skilled service. An aide-only plan of care does not meet Medicare home health eligibility.
Telecommunications HCPCS codes: G0320, G0321 and G0322
CMS permanently allowed home health agencies to use telecommunications technology as part of the plan of care, and it requires agencies to report that use on claims. Reporting became mandatory on July 1, 2023.
HCPCS | What it reports |
|---|---|
G0320 | Home health services furnished through real-time two-way audio and video |
G0321 | Home health services furnished through real-time audio-only communication, such as a phone call |
G0322 | Collection of physiologic data digitally stored or transmitted by the patient to the agency, such as remote patient monitoring |
Key rules for these codes:
Report them only with discipline revenue codes 042X, 043X, 044X, 055X, 056X or 057X.
Report each service as a separately dated line under the discipline that provided it. Two audio-video or audio-only services on the same day under the same revenue code go on separate lines.
They are informational. They carry no separate payment and are excluded from visit counts, LUPA calculations and outlier units.
An in-person visit line is no longer required on the same claim. CMS removed that edit in 2023 after it caused claims to return in error.
Because these codes do not count as visits, a period built heavily on virtual contacts can fall below its LUPA threshold. Plan in-person visits with that in mind.
HCPCS codes billed outside the 30-day period
Some services are paid separately and billed on type of bill 034X rather than the period claim:
Vaccines and administration. Influenza, pneumococcal and hepatitis B vaccines are billed with the vaccine product code and an administration code such as G0008 (influenza), G0009 (pneumococcal) or G0010 (hepatitis B).
Disposable negative pressure wound therapy (dNPWT). dNPWT devices are paid separately. For CY 2026, CMS set the separate payment amount at $282.10.
Certain Part B services and supplies furnished to patients not under a home health plan of care.
Durable medical equipment furnished as a home health service is paid under the DME fee schedule and is not included in the 30-day rate.
Supply reporting
Non-routine supplies, such as wound dressings and catheter supplies, are bundled into the 30-day period payment under PDGM. Agencies still report them on the claim using supply revenue codes such as 027X and 0623, because the data feeds CMS cost and outlier analysis. Do not try to bill bundled supplies separately to Medicare.
Per-visit payment rates tied to HCPCS disciplines
When a period becomes a LUPA, Medicare pays per visit at national rates, adjusted by wage index. The CY 2026 national per-visit amounts are:
Discipline | CY 2026 per-visit rate | With LUPA add-on (first or only period) |
|---|---|---|
Skilled nursing | $176.96 | $304.37 |
Physical therapy | $193.42 | $313.82 |
Occupational therapy | $194.74 | $335.69 |
Speech-language pathology | $210.25 | $351.03 |
Medical social services | $283.64 | Not applicable |
Home health aide | $80.12 | Not applicable |
The LUPA add-on applies to the first skilled nursing, physical therapy, occupational therapy or speech-language pathology visit in a LUPA period that is the only period or the first in a sequence.
Common home health HCPCS errors
Wrong discipline revenue code. A G0151 on a 055X line will reject. Map every G-code to its revenue code in your EMR.
RN code for an LPN visit. Billing G0299 for an LPN visit misstates who provided care.
Overusing G0493. Observation and assessment needs a documented reason for skilled evaluation.
Telehealth lines with the wrong revenue code. G0320–G0322 only belong on discipline revenue codes.
Mismatched dates. Line-item dates must match visit notes and fall within the period.
Missing the 0023 line. Every final claim needs revenue code 0023 with the HIPPS code.
Counting virtual contacts as visits. Telecommunications lines do not count toward LUPA thresholds.
Getting visit coding right every time
The simplest protection is automation plus review. Configure your EMR so each visit type generates the correct G-code and revenue code, then have billing staff review exceptions before claims drop. A dedicated home health billing service can run those reviews daily and track LUPA risk while periods are still open.
Agencies comparing options for home health billing in USA markets should ask how vendors validate G-codes against documentation. In Southern California, home health billing companies in Palm Springs must also handle Medi-Cal plans that may use different code requirements. In Texas, agencies using home health billing services in Houston need visit coding that holds up under Review Choice Demonstration review. In New York, home health billing services must reconcile visit codes with EVV data required by state Medicaid.
As a specialized Trucare Home health billing Company, TruCare maps every visit type, checks HCPCS-to-revenue-code pairing and flags periods at risk of a LUPA before they close.
FAQs
Do home health HCPCS codes determine payment? Not directly for full periods. Payment is based on the HIPPS code. But HCPCS visit lines determine LUPA status, per-visit payments and outlier calculations.
What is the HCPCS code for an RN skilled nursing visit? G0299 is used for direct skilled nursing by an RN. Other RN codes, such as G0493, G0495 and G0162, describe specific types of skilled nursing.
Are telehealth G-codes paid? No. G0320, G0321 and G0322 are reporting codes only and do not count as visits.
What revenue code goes with G0156? Revenue code 057X, for home health aide services.
